- This study examined general dentists' preferences for esthetic smile characteristics based on age and gender of the dentist and the simulated patient using a 16-item survey. Smile characteristics evaluated were smile line, incisal embrasure size, gingival display, and vertical position of the lateral incisor. Photographs showing a smile of a young female, older female, young male, and older male were taken, cropped, and mirrored at the midline using Adobe Photoshop to create bilaterally symmetrical smiles. Images of each patient were then incrementally altered for each smile characteristic. Three images were created and displayed on a single page for each patient and the given smile characteristic. Participants were recruited through email advertisement and if they met the inclusion criteria, they completed the survey in a solitary, uninterrupted manner, with the instructions to select the most esthetic smile per page. Frequency of responses were evaluated as a function of the responding dentist's gender, age and graduation year, as well as by the simulated patient's gender and age. Associations were tested using either a Chi-square test or a Fisher's exact test if the expected number of responses was small. The significance level was set to 0.05. Seventeen general dentists from UMKC School of Dentistry (faculty and AEGD residents) participated. Ten were female and seven were male. They ranged in age from less than 30 years old to 66-70 years old. Year of graduation from dental school ranged from 1976 to 2018. The only significant finding for the impact of dentist's age and gender on smile preferences was between genders within graduation year groups. Significant differences for smile line and vertical position of the lateral were found for older patients. Gingival display in female patients was also significant. For older male patients, all smile characteristics exhibited significant preferences, while only vertical position of the lateral did for the older female patient. For older patients, between genders, significant preferences were found for gingival display. The results of this study suggest that the age and gender of the dentist have little impact on esthetic smile preferences, while the patient's age and gender significantly influence smile preferences
